چکیده

This study was a prospective evaluation of serial muscle enzyme changes in 25 consecutive patients who were suffering from neck pain following a rear end motor vehicle accident. The objectives were to assess if muscle damage occurs and to quantify it following a whiplash injury. It is often stated that a whiplash injury is a soft tissue injury consisting of damage to the muscles and ligaments of the neck. However no study has looked at this. MRI scans occasionally show haematoma within the paravertebral muscles but are not sensitive enough to assess muscle damage. Creatinine kinase has been shown to be the most sensitive indicator of muscle damage Twenty-five consecutive patients suffering from neck pain following a rear end collision were recruited. Serial creatinine kinase measurements were taken on three consecutive days including the day of the accident and a control value at three months following the injury. Only patients with isolated whiplash injuries were included. There were no significant rises in creatinine kinase in any of the patients in the study, even in two patients who had to be hospitalised because of the severity of their symptoms. We have found no evidence that the symptoms arising after a whiplash injury are due to muscle damage and other avenues need to be explored.

Whiplash evokes descending muscle recruitment and sympathetic responses characteristic of startle.

Whiplash injuries are the most common injuries following rear-end collisions. During a rear-end collision, the human muscle response consists of both a postural and a startle response that may exacerbate injury. However, most previous studies only assessed the presence of startle using data collected from the neck muscles and head/neck kinematics. The startle response also evokes a descending p...

The Course of Serum Inflammatory Biomarkers Following Whiplash Injury and Their Relationship to Sensory and Muscle Measures: a Longitudinal Cohort Study

Tissue damage or pathological alterations are not detectable in the majority of people with whiplash associated disorders (WAD). Widespread hyperalgisa, morphological muscle changes and psychological distress are common features of WAD. However little is known about the presence of inflammation and its association with symptom persistence or the clinical presentation of WAD. This study aimed to...

What occupant kinematics and neuromuscular responses tell us about whiplash injury.

STUDY DESIGN Literature-based review. OBJECTIVE To review the published data on occupant kinematic and neuromuscular responses during low-speed impacts and analyze how these data inform our understanding of whiplash injury. SUMMARY OF BACKGROUND DATA A stereotypical kinematic and neuromuscular response has been observed in human subjects exposed to rear-end impacts. Combined with various mo...

Persistent pseudomyopia following a whiplash injury in a previously emmetropic woman

Purpose Accommodative spasm, which manifests as ciliary muscle spasm, convergent strabismus or miosis, is a recognised consequence of head trauma. In whiplash cases, cervical spine hyperextension poses a risk of contra-coup injury and brainstem trauma, and is known to affect the visual system. However, to date, no cases of accommodative spasm due to whiplash injury have been reported. Observa...

Antinociceptive reflex alteration in acute posttraumatic headache following whiplash injury.

Brainstem-mediated antinociceptive inhibitory reflexes of the temporalis muscle were investigated in 82 patients (47 F, 35 M, mean age 28.3 years, SD 9.4) with acute posttraumatic headache (PH) following whiplash injury but without neurological deficits, bone injury of the cervical spine or a combined direct head trauma on average 5 days after the acceleration trauma. Latencies and durations of...
